Overview

The "Incident Investigation" course is a practical occupational safety and health program designed to build structured competence in workplace incident investigation, evidence collection, root-cause analysis, professional reporting, and corrective-action tracking. The program combines technical knowledge, recognized international standards, local compliance awareness, operational checklists, and applied workplace examples.

  4. Use root-cause-analysis tools such as 5 Whys, Fishbone, Fault Tree Analysis, and Change Analysis.
  5. Classify immediate, contributing, and root causes and identify weaknesses in management systems.
  6. Prepare a professional investigation report and SMART recommendations with follow-up responsibilities.
